چکیده

T he etiology of asthma is not known. Acute and chronic asthma can be triggered by several factors, including allergen exposure, upper respiratory tract infections (particularly viruses), exposure to irritants such as cold air, cigarette smoke, and other air pollutants, exercise, nonsteroidal antiinflammatory drugs, sulfites, and possibly emotional factors.’ While the exact role of allergen exposure in asthma is unknown, 75% to 85% of patients with asthma have positive immediate skin test reactions to common inhalant allergens.2 These and other observations demonstrate a strong and well established association between asthma and allergic disease. These figures may overestimate the true role of allergen exposure, but indicate the need for evaluation of the role of allergen exposure in the diagnosis and treatment of asthma. The extent to which allergy triggers asthma varies somewhat according to the age of the patient. In infants who have not yet become sensitized to aeroahlergens, respiratory infections are more likely to initiate asthma. In children beyond infancy, allergy assumes importance not only in persistence and severity, but also possibly in the development of the disease. Studies have reported that the severity of childhood asthma correlates with the number of positive skin tests.34 Children who have multiple positive skin tests may have asthma symptoms on a daily rather than intermittent basis.’ Exposure to aeroallergens may be important in adults under age 30. Occupational exposure may also account for up to 2% of all asthma.6 In contrast to findings in children, the presence of allergen skin test reactivity in adults does not appear to be associated with increased severity of asthma.

Management of Rhinitis: Allergic and Non-Allergic

RHINITIS IS A GLOBAL PROBLEM AND IS DEFINED AS THE PRESENCE OF AT LEAST ONE OF THE FOLLOWING: congestion, rhinorrhea, sneezing, nasal itching, and nasal obstruction. The two major classifications are allergic and nonallergic rhinitis (NAR). Allergic rhinitis occurs when an allergen is the trigger for the nasal symptoms. NAR is when obstruction and rhinorrhea occurs in relation to nonallergic, n...

Consensus Definition of Nonallergic Rhinopathy, Previously Referred to as Vasomotor Rhinitis, Nonallergic Rhinitis, and/or Idiopathic Rhinitis

"Nonallergic vasomotor rhinitis" (also referred to as nonallergic rhinitis and/or idiopathic rhinitis) is a term that has been used to describe a common nasal condition of unclear pathophysiology. The lack of straightforward diagnostic criteria is limiting; research for better treatment options requires the definition of homogeneous populations characterized by well-defined inclusion and exclus...

Newer Insights into the Prsevention of the Common Cold

Since most colds are self-limiting, symptoms will usually resolve on their own in seven to ten days. For the majority of patients, treatment is symptomatic and targeted at the most bothersome symptoms. Specific nonprescription therapy may relieve cold symptoms but should be used with caution, especially in at-risk patient populations. Patients should monitor for worsening symptoms, and progress...

Distinct macrophage phenotypes in allergic and nonallergic lung inflammation.

Chronic exposure to farm environments is a risk factor for nonallergic lung disease. In contrast to allergic asthma, in which type 2 helper T cell (Th2) activation is dominant, exposure to farm dust extracts (FDE) induces Th1/Th17 lung inflammation, associated with neutrophil infiltration. Macrophage influx is a common feature of both types of lung inflammation, allergic and nonallergic. Howeve...

Consensus Description of Inclusion and Exclusion Criteria for Clinical Studies of Nonallergic Rhinopathy (NAR), Previously Referred to as Vasomotor Rhinitis (VMR), Nonallergic Rhinitis, and/or Idiopathic Rhinitis

"Nonallergic rhinopathy" was defined by consensus at a Roundtable conference in December 2008 as "a chronic nasal condition with symptoms that may be perennial, persistent, intermittent or seasonal and/or elicited by recognized triggers." The definition includes a well-recognized set of clinical exposures that lead to the symptoms, predominantly congestion, rhinorrhea, and postnasal drip. These...
